Nurse Practitioner (NP) - Full-time Career Opportunity
Job Summary:
Reporting to the Executive Director, the Nurse Practitioner (NP) primary health care, including disease management and prevention, health promotion, health assessment and therapeutic interventions to Sioux Lookout area residents. The Nurse Practitioner is an integral part of the patient's circle of care with advanced clinical assessment, diagnosis, and treatment plan. The Nurse Practitioner will engage in providing primary health care services to individuals across the lifespan.
Qualifications:
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (EC)
- Bachelor of Science in Nursing degree
- Completion of the Primary Health Care Nurse Practitioner Program
- Sound clinical background and strong health assessment and diagnostic skills
- Demonstrated ability to work collaboratively as part of an interdisciplinary health care team
- Current CPR Certification
- Sensitivity to cultural, religious, language and gender differences
Key Responsibilities:
- Work within the Nurse Practitioner's scope of practice as set out by the College of Nurses of Ontario Nurse Practitioner Standards
- Perform comprehensive health assessments within the scope of the Nurse Practitioner's role
- Formulates and communicates medical diagnoses
- Provides acute and episodic care
- Liaise on and collaborate with families and community stakeholders
- Consults with a physician in accordance with the standards for consultation with physicians, and/or refers the patient to another health care professional
- Recommends and facilitates referrals to other services as well as communication and networking between the primary care practice and the hospital.
- Maintains relationships with networks, related professional organizations, and other related service providers
- Maintains privacy, confidentiality, and security of patient information at all times.

Salary range and Benefits – $130,000 - $150,000 per annum based on qualifications and experience. Full health benefits, dental, vision, sick leave and GRSP.

Dietitian - Part-time Virtual Career Opportunity
Job Summary:
Reporting to the Executive Director, the Registered Dietitian will provide assessments, consultations and the development of treatment plans for patients. The Registered Dietitian will work along side physicians to provide care and resources related to nutritional needs for patients.
Qualifications:
- Bachelor's Degree in Applied Nutrition or equivalent
- Registration with the College of Dietitians of Ontario
- Minimum 2 years experience as a Dietitian or Nutritionist
- Knowledge of health challenges in rural Northwestern Ontario
- Excellent written and verbal communication skills
- Sensitivity to cultural, religious, language and gender differences
Key Responsibilities:
- Provides resources to patients in regards to nutritional needs on individual basis
- Complete patient assessments and work with health providers in the community to recommend the best plan of care for patients
- Practice on an individual basis or as a member of an interdisciplinary team to determine nutritional needs of patients and to plan, implement and evaluate normal and therapeutic diets to maintain and enhance general health
- Evaluate nutritional status of individuals and aid in the prevention and/or treatment of inadequate nutrition
- Provide nutrition guidance, label interpretation and consultation services
- Maintains relationships with networks, related professional organizations, and other related service providers
- Maintains privacy, confidentiality, and security of patient information at all times.
